Innovative Health Care Delivery Model 
There are different healthcare delivery models all aimed at improving the quality of healthcare services for the citizens of America. These models are also meant to reduce the cost of healthcare delivery as high costs of healthcare provision may affect the delivery of quality healthcare and in the achievement of other healthcare goals (Spruce, 2015). Innovative healthcare delivery models are aimed at improving the quality of healthcare services, which will also increase patient outcomes. This is achieved through the engagement of nurses in the different healthcare fields (Spruce, 2015). One of the innovative healthcare models that incorporates an interdisciplinary care delivery team is the Patient-Centered Medical Home Delivery Model (Giomuso et al., 2014).

This model was determined in the Affordable Care Act and is a model, which is meant to provide hospital level care at traditional home settings (Giomuso et al., 2014). This model focuses on preventive and chronic care, which can be extended at homes, which reduces emergencies and acute care cases in healthcare facilities. Nurses are important in the implementation of this model as they are able to provide more information regarding patients since they interact more with patients (Giomuso et al., 2014). Nurses have also adequate knowledge on patient evaluation and patient education, which makes them a vital component of this model. A primary care physician effectively coordinates this model and patients at home are able to receive healthcare services from nurses. Complications related to chronic illnesses can be avoided by using this model, as patients can be cared for while at their homes (Giomuso et al., 2014).
This will reduce the costs incurred in accessing medical services as well as improving the healthcare outcomes (Zajac, Norris, & Keenum, 2014). This model also allows patients to seek healthcare services in time, which improves their recovery process, as there are available staff members ready to help the patient (Zajac, Norris, & Keenum, 2014). For patients having chronic illnesses, this model allows the patients to have support all the time, which helps the patients to recover quickly from any dangers affecting them. The patients are also able to interact more with the responsible staff members, which increases trust and mental satisfaction, which is important in the recovering process of patients (Zajac, Norris, & Keenum, 2014).
